Multimodal Dangerous Goods Form (MO41)
All shipments of hazardous goods must have the shipper or packer complete the MO41 Declaration form prior to booking requests with Shipping Lines. This must include Inner and Outer Packaging types along with Gross and Net weights.
MSDS (Material Safety Data Sheets) must also be lodged in support of this declaration.
